Event Submission Form - ALC

Please use this form to submit a request to use NSUMC church facilities to hold an event. If you wish to promote your event, please submit all information at least 4-6 weeks in advance so we can best facilitate scheduling. If you have any questions or would like a physical copy of this form, contact the office at (480) 948-0529 or office@nsumc.com.

Event Details

Please include the name and a detailed description of the event. This information will be used to assist in marketing and promotional efforts, if applicable. If you have promotional art that you wish to use, please email it to office@nsumc.com.

Date

Please select all rooms/spaces that you wish to utilize during your event. You only need to check "Parking Lot" if you plan to use the space in an unconventional way (i.e. food trucks, tents, etc.).
